The 3-year-old bay colt was trained by the legendary Bob Baffert, who with his seventh win becomes the most successful trainer in the history of the Derby. Hall of Fame jockey John Velasquez scored his fourth Run of the Roses win.
Medina Spirit went into the derby with the sixth best odds at 12: 1 and won second place at Mandaloun, third place at Hot Rod Charlie and the derby favorites Essential Quality in fourth place.
The participation in the derby was 51,838. NBC commentators said the derby is considered the most-attended sporting event in the United States since the coronavirus pandemic began.
The Preakness Stakes, the second race in the Triple Crown, is scheduled for May 15 at the Pimlico Race Course in Baltimore, Maryland.
